Community Living Supports – Family Model is a service designed to provide seniors and adults with disabilities an alternative living arrangement. In this model, individuals live in the homes of specially trained family caregivers. CLS-FM offers hands-on assistance, supervision, transportation, and other support services. The primary goal is to help individuals transition from nursing home living to community living or to prevent the need for nursing home placement altogether. CLS-FM services provide all the necessary supports to enable individuals to live successfully in their community. This assistance helps them acquire, retain, and improve the skills needed to thrive in a family environment, specifically in the homes of trained caregivers who are not their relatives. Our caregivers undergo a thorough recruitment, screening, and training process before providing any services. Assistance is available 24/7, and each home is limited to no more than three supported individuals.
The services provided include direct support with:

- Essential household chores for the health and safety of the supported individual

- Attending appointments
- Building interpersonal and social skills to thrive in a community home
